2. Medicare Part D Prescription Drug Plans

To find the best prescription drug plan you must go to www.medicare.gov, enter the drugs that you are taking along with the proper dosage.  Medicare’s website will list all of the plans that cover your prescription drugs in order of annual cost.

Often times you can reduce the cost of your prescription drugs by coordinating the Part D plan with the Alternate Prescription Drug Plan above.

If you would like help selecting the best combination of the two plans please follow the instructions below.

General Instructions:

Prepare a complete list of all the prescription drugs you are taking along with the proper dosage and correct spelling.

A new list should be submitted every year around November 15th because (1) your prescription needs may change (2) generic drugs may become available (3) there may be new approved name brand drugs and (4) Part D Prescription Drug Plans may change every year.

When we receive your Prescription Drug List, we will use Medicare’s Website to determine what plan will best fit your needs.  Also, we will coordinate Part D with alternate drug plans to make sure you are able to purchase your drugs at the lowest possible cost.

Example:

Lipitor   10mg   1 / day
Fosamax   70mg   1 / week
